shoulder rotor cuff problem or strain

Suggestions to repair or help healing.

I have been to physical therapy for several months and no improvement. I try to work it as much as I can on the workouts that I schedule. Right now it aches the crease, under the forearm and my bicep. It was getting better I thought with my workouts but I think it is set back due to the weights. I am only using a 3 and 5 lb weight when it calls for weights. Any suggestions would be appreciated.
