Why can’t i stick to working out?? I keep failing

I honestly am getting so frustrated with my self, i start and stop i’ve been doing it for 5 years i can’t believe so much time has gone by… I got my dream healthy body in 2016 with just fitness blender workouts. i started a really demanding job let myself go and smoked lots of cigarettes i only fully quit six months ago. Now that i have my lungs reasonably healthy again now i just feel like i can’t get myself to just stick to it for more then 4 weeks and then i just say whatever and eat whatever and how much i want till it’s get to the point where i literally feel like a disgusting human and can’t bare to look at myself in the mirror then the cycle starts all over again…
