Cooked vs. uncooked for meal plan measurements

I'm trying to figure out in the meal plan if grain measurements are for cooked or uncooked. For example, "Brown rice, cooked" or "Penne, cooked". I noticed in the pasta recipes the instructions specify to cook the pasta, but I'm not seeing that in other places for rice. I think traditionally "1 cup brown rice, cooked" means you measure 1 cup dry rice then cook it, whereas "1 cup cooked brown rice" means you cook first and then measure out one cup. The meal plan intro does say that the recipes specify cooked or uncooked, but I'm still confused.
