Neck soreness during upper body workouts

Hey everyone!

I just completed an upper body workout last night and while I was doing some of the exercises (lateral/ventral raises and overhead press) my neck and traps felt like they were on fire! When I woke up this morning my neck and traps are sore and stiff. This seems to happen to me a lot. Some times the next day I'll wake up with a headache too.

My question is what (if anything) am I doing wrong? Improper form? Lifting too heavy? Has anyone else experienced this?
