Lifting heavy

I'm using a set of Powerblock dumbbells for strength training. The weight increases in 4 lb increments. For some exercises, I can complete the set without struggling too much with, let's say, an 8 lb weight. If I go up one increment, to 12 lb, I may only be able to do 3 or 4 reps instead of a full set. Am I adding too much weight initially or should I continue with the initial low number of reps, gradually building up to a full set? I'm asking coz K&D always say "the last 8-10 reps should be hard". In this scenario, reps 4-5 are hard!
